These forms may not be sent by email, as email is not a secure method for transmitting health information. Completed forms must be submitted to the Health Center in one of the following ways:
- Uploaded through your secure patient portal
- Returned in person prior to your first appointment
- Faxed to the Health Center allergy nurse at 814-898-6924
- Mailed to: Health Center, Penn State Behrend, Carriage House, 4701 College Drive, Erie, PA 16563
